Is Cas getting the warning to not look back due the holes being punched in reality? He gets a little fixated on them later, so is instinct telling him don't look back and get fixated on the strange phenomenon happening behind you?

I’m pretty sure this has to do with the Contract which Cas signed but has no memory of. Because he doesn’t seem to know what is going on but instinctively is able to fulfill the task and he doesn’t recognize the pretty clear signs of being under a compulsion in the moment (contrast that with Dean’s compulsion later in book 1). Also because as Dean notes, he did something to injure his hand before the start of the chapter but neither Cas nor the audience knows what it was yet (and these boys are always slashing open their palms when they need blood for spellwork).

This is my third re-read and I JUST got that "don't look back" and "I don't want to hurt you" wasn't Dean talking to Cas. I didn't even register that his hand was injured by something other than the fight. This is an awesome explanation, thank you!!!

Oooh! I'd forgotten about the contract. You're right though, it fits with the hand injury which I always took for granted as fallout from the episode.
